We are an outpatient specialty clinic devoted to caring for the needs of veterans as they age, working together with primary care and other subspecialists to focus on maximizing independence, managing medical conditions associated with aging, such as cognitive impairment/dementia, delirium, frailty and falls. We perform medication reviews to help simplify medication regimens, and together with our colleagues in palliative care, specialize in advanced care planning and end of life planning discussions. We are an interdisciplinary team including Geriatric trained physicians, Nurse Practitioner, Social Work, Neuropsychology, Pharmacy. We are a teaching site for Harvard Medical School and are affiliated with the New England GRECC (Geriatrics Research, Education and Clinical Center).
