Geriatrics Clinic

The Geriatrics clinic serves older and frail veterans. We are an outpatient specialty clinic devoted to caring for the needs of veterans as they age, working together with primary care and other subspecialists to focus on maximizing independence, managing medical conditions associated with aging, such as cognitive impairment/dementia, delirium, frailty and falls. We perform medication reviews to help simplify medication regimens, and together with our colleagues in palliative care, specialize in advanced care planning and end of life planning discussions. We are an interdisciplinary team including Geriatric trained physicians, Nurse Practitioner, Social Work, Neuropsychology, Pharmacy. We are a teaching site for Harvard Medical School and are affiliated with the New England GRECC (Geriatrics Research, Education and Clinical Center). Our team is involved in exciting clinic, research and educational efforts to improve care for aging veterans!

What Should A Veteran Bring With Him/Her Before Coming To Their Appointment Or What Should They Do Before Being Seen?

Please bring all medication bottles and pillboxes, a photo ID (such as your Veteran’s ID card or a driver’s license) and your health insurance information. We encourage you to bring a family member or caregiver if applicable. If you are transferring from a health care provider outside the VA, it is helpful if you bring your medical records with you.
